A referral is a written order from your primary care doctor for you to visit a specialist or speak with a certain medical service. In many Health Maintenance Organizations, also known as HMOs, you need to get a referral prior to receiving the medical care from anyone except for your primary care doctor. Step 1: Visit Our Referral Page
After speaking with your primary care doctor and receiving their consent, visit our Referrals page on our website at https://www.uhsma.com/referrals/.
Step 2: Downloading Our Form
From there, you will use the link on the page to download our referral form.
Step 3: Filling Out The Form
The information that you will need to include on our referral form mentions the following:
date, client name, date of birth, address, telephone, email, primary insurance, insurance id#, group # (if applicable), referral source, referral telephone, and the reason for referral.
Step 4: Submitting The Form
Once completed, please submit via scan and email or fax at (617) 203-6651. Once you have sent over the referral form, we will be in contact with you shortly.
